INSURANCE

I am an out of network provider for all insurance companies.  Services may be covered in part by your insurance under your out-of-network benefits.  Please contact your insurance provider to verify coverage for out-of-network providers.  I will gladly provide a "superbill" with all of the necessary codes that you need to submit to your insurance provider for reimbursement.  

​Here are the procedure (CPT) codes I most frequently use:

90791 for the initial diagnostic 75-min session

90834 for weekly 45 min sessions

​90837 for weekly 60 min sessions

​Payment is due at the time of service. I accept credit cards, HSA, and payments through Zelle.  All billing paperwork will be managed on my electronic health record platform, Sessions Health.

CANCELLATION POLICY

If you are unable to attend your schedule appointment for any reason, 24 hour notice is required in order to avoid being charged the full session rate.
