INSURANCE

If you are unable to attend your scheduled appointment, for any reason, 24 hour notification is required.  Missed or cancelled appointments with less than 24 hour notice will be charged the full amount of the scheduled session.

I am an out-of-network provider for all insurance companies.  Services may be covered in part by your insurance under your out-of-network benefits.  Please contact your insurance provider to verify coverage for out-of-network providers.  I will gladly provide a "superbill" with all of the necessary codes that you need to submit to your insurance provider for reimbursement.  An explanation and copy of your Good Faith Estimate will be provided upon starting services with me. 

​If you want to check in advance to see if your insurance company provides coverage for therapy with an out-of-network provider, here are the procedure (CPT) codes I will most frequently use:  

90791 for the initial diagnostic session, 90834 for weekly sessions.

Payment is due at the time of each session.  I accept payments by credit/debit cards via my online portal, Simple Practice, which clients will be enrolled in upon starting services with me.
